Please provide us your name as shown on your passport. You do not have to be a certified diver to travel with a group. We provide diver certification classes at the destination, and in most instances, a certification will be issued to you. Submitting this form will book your seat however we will not be responsible for overbooking in which case you will be refunded any deposits or payments. Travel insurance is highly recommended in case of emergency cancellations by you. You will receive an electronic copy of this form for your records. We are an IATA agent. Thank you.
